Commonly asked questions from employers and plan administrators

Simply complete this form, and a Colonial Life representative will contact you regarding our Premium Only Plans.

To file a claim, policyholders should click on log in the upper right corner of the website and enter their login information. Once logged in, click on Claims Center. From there, simply click the File an Online claim button to file many types of claims such as: wellness, doctor's office visits, accident, pregnancy, sickness and vision claims. They may also download claim forms and find answers to many claims-related questions.
The ability to maintain coverage after an employer stops deductions is determined by the policy and/or through our administrative guidelines. We recommend that policyholders complete a Conversion Form and return to our office for processing. Policyholders may view a copy of their policy by logging in to their online account and by clicking on My Correspondence.
To make a premium payment, policyholders should click log in in the upper right corner of the screen and enter their login information. Once logged in, click on Pay My Bill then Make a Payment. They will be brought to our secure payment form where you can enter their payment information and even set up recurring payments.

To view a claim, policyholders should click log in in the upper right corner of the pages on our website and enter their login information. Once logged in, recent claims will be available on the home page, or they can access them via the Claims Center.

To view policy status, the policyholder should click log in in the upper right corner of the page and enter their login information. Once logged in, their policy status will appear on the home page.

Click on register in the upper right corner of the page, then follow the instructions to complete the form on the next page.

The ability to continue coverage during a leave of absence is addressed in the policy and/or through our administrative guidelines. If coverage can be continued during a leave of absence and the policyholder's employer will continue to send premiums to Colonial Life, no action is necessary.

If the policyholder's leave of absence is three months or less and their employer will not be sending their premiums to Colonial Life, the policyholder needs to send their premium payment to Colonial Life. They'll need to send the amount that would have been deducted from their paycheck for the pay periods they'll miss.

In addition to sending their premium payment, if they'll be on a leave of absence for three months or more and their employer will not be sending their premiums to Colonial Life, we require that they complete and send us the Request for Service form to change their policy to be paid on an individual payment basis.

Simply fill out this form and a Colonial Life sales representative will contact you.

To view their policy, policyholders should click log in in the upper right corner of the page and enter their login information. Once logged in, click on My Correspondence, then View Correspondence. From there, click on the policy they wish to view. NOTE: if they have not viewed their policy recently, it may be archived. If they receive a notice indicating their policy is archived, please wait a few minutes and try again.
